Employees wishing to make changes to their benefits for 2008 must do so within the Open Enrollment time period listed above.
Flexible Spending Account: Employees who wish to be enrolled in a Flexible Spending Account in 2008 must enroll/re-enroll during Open Enrollment.
NEW EMPLOYEES: You must enroll in your benefits within 31 days of your date of hire. Failure to enroll within this time frame may result in you forfeiting your right to enroll in many of the benefits until Open Enrollment.
Effective 8/1/07 accounts administered by PARS in the 457 Deferred Compensation Plan will be moved to JEM Resource Partners for administration. This will not affect the investment options available, but it will prevent you from receiving a distribution or being able to reallocate your funds during the 31 day blackout period starting on August 1, 2007.
First Time Users
Your user ID is the first six digits of your last name followed by the first initial of your first name followed by the last four digits of your Social Security Number.
Example: John T. Jones, SSN 123-45-6789 User ID: jonesj6789
(Please note: your user ID may be shorter if you last name has few than 6 letters)
Password: Use your Social Security Number, with no dashes or spaces:
Example: John Jones’s Password would be: 123456789
PLEASE NOTE:
- You will be able to view your benefits profile year round, so keep your user ID and password in a safe place
- You MUST print your benefits summary once you have made the necessary changes. Keep this for your records and as proof of any changes you made.
BEFORE YOU SIGN IN:
- Make sure you have the names, birthdates and Social Security Numbers of any family members you wish to cover under the District’s benefits programs.
- If you are signing up for the AETNA DMO Dental Plan you must have your Dental Provider I D #. Click here for a partial listing of local dentists in the plan to obtain their ID #. If you do not find your facility here you may look up their ID # at www.aetna.com and click on the “Find a Physician” feature.
ELIGIBLE EMPLOYEES
Effective January 1, 2007, any CCISD permanent employee (not seasonal or substitute) who contributes to TRS or otherwise would contribute to TRS if they were not a TRS Retiree is eligible to enroll in all of the District’s insurance benefits. This includes half-time paraprofessional, auxiliary and professional position who were previously excluded from the Basic Life Insurance, Long Term Disability, and Dental Insurance.
ELIGIBLE DEPENDENTS
For the District’s medical, dental, vision and life insurance plans, eligible dependents include your spouse and children who are unmarried, under age 25, and financially dependent upon you for at least 50% of their support. Stepchildren residing with you, grandchildren living with you in a parent-child relationship, legally adopted children and children for whom you have legal guardianship or managing conservatorship and who meet the requirements for natural children may also be covered
