Employees wishing to make changes to their benefits for 2012 must do so within the Annual Enrollment time period listed above.
FLEXIBLE SPENDING ACCOUNTS
Employees who wish to be enrolled in a Flexible Spending Account in 2012/2013 (September 2012 to August 2013) must enroll/re-enroll during Annual Enrollment.
NEW EMPLOYEES,
You must enroll in your benefits within 31 days of your date of hire. Failure to enroll within this time frame may result in you forfeiting your right to enroll in many of the benefits until Annual Enrollment.
LOGIN INSTRUCTIONS
Your Username Is:
The first Six (6) characters of your last name, followed by the first letter of your first name, followed by the last Four (4) digits of your Social Security Number.
Your Password Is:
Your password is the first letter of your first name, followed by the first letter of your last name, and the last four numbers of your Social security Number.
REMINDERS

You will be able to view your benefits profile year round, so keep your user ID and password in a safe place.

You are strongly advised to print your Benefits Summary once you have made the necessary changes. Keep this for your records and as proof of any changes you made.

Make sure you have the names, birthdates and Social Security Numbers of any family members you wish to cover under the District’s benefits programs.

If you are signing up for the AETNA DMO Dental Plan you must have your Dental Provider I D #. You may look up their ID # at www.aetna.com/docfind
ELIGIBILITY FOR HEALTH BENEFITS
You will be eligible for health insurance benefits if you are an active, contributing TRS member or employed 10 or more regularly scheduled hours each week.
Employees may cover the following dependents:
- Spouse , and
- A child under the age of 26, such as:
- A natural or adopted child
- A stepchild
- A foster child
- A child for whom you have legal guardianship
- A grandchild under the age of 26 whose primary residence is the household of the employee and who is a dependent of the employee for federal income tax purposes for the reporting year in which coverage of the grandchild is in effect
- Any other unmarried child in a regular parent-child relationship with the employee, meaning:
- The child’s primary residence is the household of the employee;
- The employee provides at least 50% of the child’s support
- Neither of the child’s natural parents resides in that household;
- The employee has the legal right to make decisions regarding the child’s medical care
